2024-25 Student Learning Experience Questionnaire

1. Students will first visit a newly created SLEQ landing page on our faculty website.
2. They are expected to read the “INFORMATION COLLECTION AND PARTICIPATION CONSENT STATEMENT” on the second page, and check the box “I have read and agree to the above statement.” if they wish to join the survey.
They will be directed to the SLEQ student login page.
3. Survey participants will automatically be eligible for receiving the coupon if they submit the completed survey form. There is no need to indicate their preference of “opt-in” or “opt-out” the gift exercise.

4. Students can simply quit the SLEQ landing page if they do not wish to join the survey.
